How do I report days away from work on OSHA 200 in the following case?

An employee reported on 12/11/01 that he injured his shoulder on 7/1/01 (Yes, that is bad, but not my immediate problem). He had not missed any work but he had received cortisone shots in Sept.

On 12/14/01, the same employee reported a knee injury which occured on 12/13/01.

The employee is now off work, probably because of the knee but even if the knee was well the shoulder is now bad enough he still couldn't work. Do I report the days off under one injury and leave the other blank? If yes, which injury?

Unless the doctor's report specified that the days away from work are due to the shoulder, record the lost days for the knee injury. It's the last reason he received medical care. If you find out later that the shoulder was the cause of the lost days, it's okay to make corrections to the 300 Log. Also, the new recordkeeping rules call for counting [u]calendar days[/u] for days away from work up to a maximum of 180 days.

I was told to use the OSHA 200 log and rules for year 2001 and the 300 log and new rules start with cases reported after 1/1/02. Is this correct?

Yes, you are right. I failed to note the dates in 2001.

I am responding a tad late, however, if this employee received medical attention to his/her shoulder (cortisone shots), then this also needs to be recorded even if there were no days missed.
