Hopefully my last question!

When an ee needs FMLA, they request time off. I have several forms here: One form; er response to ee request for FMLA states "you will be required to furnish medical certification of a serious health condition." How do I complete this form when I have to have the medical certification form completed before I will excuse the leave as FMLA?

Should I leave myself open to "I will count your leave as FMLA, unless you cannot get a certification?"

Wow, I am confused.

Thanks.

  • You don't complete it yet. Give all the forms to the employee and tell them to return them to you with the physician's certification form. Either that or tell them to complete the ee portion and give it to you, then return the medical part to you and you will provide them your answer in two days following that.
