Worker not returning on release date?

I have a worker who we had to call an ambulance on last Thursday. She came back to work on Friday but left early. She is scheduled for a stress test and some other cardio testing. She came in on Monday to pick up her FMLA paperwork, she hung around the office at our break area for about 2 hours. Well she's back at work today with a release that released her to come back to work yesterday.

A few questions:
Can I write her up for not coming in yesterday?

How long do I give her to get her FMLA paperwork back to me? This is the certification of the healthcare provider form that I am referring to.

If this person was released to come back to work yesterday and didn't show up until today, I'd write it up as a violation of your attendance policy (unscheduled absence).

I request that individuals get paperwork back to me within 15 days.
