Intermittent FMLA complicated by auto accident

I have an exempt employee, placed on FMLA for serious health condition, who returned to work for 4 hours each day. She worked for two weeks before being involved in a serious car accident. The accident caused whiplash and her physician wants her off work for two weeks.

I will count the time off toward her FMLA leave. Should I complete additional documentation or require new certification?
