FMLA & Absenteeism

Due to issues around absenteeism and tardiness, we are rewriting our very weak and vague policy to include some sort of numerical counter for these occurences. Does anyone have a recent policy that accomplishes this and still deals with FMLA, ADA, Workman's Comp? We are a non-profit with 60 employees.

As an aside, does anyone have a good method for tracking tardiness that does not involve time-clocks?
