I received an application for an intermittent leave of absence in which the doctor writes "not presently incapacitated. episodes of incapacity can occur up to ten times per year for 1 to 30 days." Doctor does not answer question about needing additional treatments. How do I answer this?

Did he answer whether this was a chronic condition? If you do not feel that you have enough information to make an educated decision, inform the employee that the paperwork has not been completed properly and have the employee take it back to the physician.
