Counting FMLA time

We do not offer any paid sick time. Employees get paid their vacation hours on their anniversary date. I have an employee that is about to exhaust 12 weeks of FMLA designated time. I am sure that she is not coming back to work. She is receiving STD from the company, and I am sure she will be eligible for LTD also. I want to know if I have to give her any more than the 12 weeks because she was eligible for vacation time. I did designate that her vacation and FMLA time would run concurrently, but I am concerned that I should give her more time because of the vacation accrual. If this employee had taken her vacation and then got sick, would I still have to give her 12 weeks plus the vacation time off? Since she did not take vacation, do I have to give her the vaction days and the FMLA time? I want to help her, but I don't want to set any precedents that will come back to bite me later. At the end of the twelve weeks, she will be paid for any vacation time accrued while she wsa out on FMLA. I am assuming that she would still accrue time. This is so confusing. Thanks for your help.
