We have a pregnant employee who has been with us four months. Does not qualify for sick leave, vacation or FMLA. She has requested 12 weeks maternity leave. Do we have to reinstate her?

Otherwise, treat it the same as you would a male with a broken leg wanting the same amount of time off. If your policy deals with it, fine. If not, disallow it.
