How often is certification needed?

You know how you think you know the answer until someone challenges you on something? Well, I thought that if an employee needed FMLA the employee needed to certify each year. For example, if I go out on FMLA today, I need a Physician's Certification Form. I will have a maximum of 12 weeks to use for this medical condition (and or any other qualifying event that may arise during the year) for the next year. If I have an on-going condition, then this time next year, I would need to re-certify. Correct?
Is there any law in California that states if someone has a life-long condition that is a qualifying event, they need to certify only once, or every couple of years?

Without new certification I would have no way of knowing your reason for absence was related to the same serious health condition as previously certified.
It seems the likelihood of this occuring would be slim. Yes?
There are some peopel who have nver certified. What a mess.
