What constitutes and employer being placed on notice of a qualifying event.

I have an employee who has been taking intermitent leave to care for her parents. although she has not asked for FMLA, it may be possible that this is a qualifying event. As the employer do I need to consider my company as having been put on notice? And if so, is it my responsibility to approach the employee and ask her to have the absences certified or is it the employees responsibility to initiate certification.
