Check off sheet for FMLA leave

Somewhere I have seen a "checkoff" sheet that employers can use to make sure they have explained all the details to employees taking FMLA leave. I believe there was even a place to note the amount of Medical Insurance payments and when they were due. Does anyone know where I can find this sheet, or am I just living in an ideal world?
