Return To Work Certificate-Samples?

Does anyone have a Return To Work form they use when employees have been out on an FML for their serious health condition, or for other medical related absences? Some employers might refer to it as a Fitness For Duty Certificate.

I am working to develop such a form for my current employer, and would be interested in examples others have found particularly successful/useful.
