Destruction of files

We are shredding files of anyone terminated 7 or more years ago. I know I need to keep a record of everyone who was destroyed, the date and what was destroyed, but do I have to list every document separately or could I do something like this:

Name
Jane Doe

Date
06/11/08

Documents shredded
Personnel file
I9
medical records

If your state has any specific details that must be retained, they would be described in state regs. Otherwise, consider keeping the following details in addition to or in lieu of what you have chosen: dates of hire, position held, exempt/nonexempt status, FT/PT status, contact info.
