Year-end benefits statement to staff

This year I want to send a year-end statement to staff showing what their true earnings were by including the benefits that the company has paid on their behalf (i.e. Medical/Dental/Life/
401k/STD/LTD.) Does anyone have a form letter/memo that you use for the year-end? Would you be willing to share with me? my email is [email]hr@youtheastsideservices.org[/email]

Hi, Marian. Mind if I tag along on your post? I posted a similar request in the "Benefits" catagory, but you might get more traffic here. I too am lookng for software or a form to use to create these things.

Our (Company) President asked the same thing the other day. I would love to know if anyone uses any software or has a form letter they use for this. (I have gotten some flyers from websites that do it and software too, but know who to trust.)
