Employee Attitude Survey & Demographic Data

Dear Colleagues,

A separate page on our survey asks for one's sex, age, function & management responsibility. The heading of the page reads: "The following questions are included exclusively to provide a basis for statistical evaluation of the various groups of respondents." Demographic page is standard boilerplate format & will be used by the agency which developed the tool & may also provide insights of attitudes of our population. A memo will be disseminated prior to the survey being conducted, which will explain the purpose of the survey & a statement that participation is voluntary. My question: Do you find the heading sufficient, or would it be best to strengthen the statement by including a notation that answering these demographic questions is strictly voluntary & no employment decisions will be made using these data?
