Vendor Insurance Certificates

Hi, I'm new to HR and have lots to learn as I do not have formal training, so I may be leaning hard on all of you! Right now I am looking for help on a template for requests for insurance certificates from vendors. Also need guidelines as to when to request that we are named additional insured. We provide engineering consulting and use drillers frequently. I think we should be named additional insured on these certificates. Any guidance would be appreciated. Thanks.
