Release of PHI - HIPAA

I work at a long term care facility for children. There are some children who's parents may be divorced or not married for some reason where one parent is the legal guardian. Am I correct in the assumption that if the legal guardian wants the other parent to have Personal Health Information about their child we will have to have an Authorization Form since this non-guardian parent is not a legal guardian even though it is their child?
