Appraisals

The HR dept. (all 2 of us) at our small Construction Co. has been tasked with putting together an appraisal form. This particular form would be given to 3rd tier employees (crewleaders and office staff) to evaluate 2nd tier employees (foremen and supervisors). If your company is currently using an appraisal form such as this I would appreciate any input you might have.
