HIPAA?? Sponsored health plan

I've read so much abouat HIPAA, it's confusing. I've read about who it covers and the terms self-insured, fully insured and employer-sponsored health plans. What is the difference? We offer medical, dental and vision coverage and pay for 70%, the employee pays 30%. I'm trying to figure out if we fall under the HIPAA regulations.

If you're self insured you own the data and are therefore responsible for HIPPA compliance. If you are fully insured (you purchase coverage for employees from a vendor) the vendor is responsible for compliance.
