We file in the personnel files. Can't really see any reason not to. There are enough other files we have to keep up with rather than making yet another one!
Our individual personnel files are sorted by 3 folders - one for training info, one for performance appraisals, corrective action notices and salary/promotion change sheets, and another to hold everything else. Medical/FMLA info is kept in a seperate file drawer. All of our folders are color coded to make it easier when filing.

Then, for those with bad appraisals who have paid me the requisite 20 bucks, the evaluations find their way to the shredder. (just kidding! Really!)
