Performance Evaluation Forms

We are in the process of revising our Performance Evaluation forms. I am interested in seeing evaluations that describe the level of performance in each category...for instance, if you are rating "Interpersonal Skills" and you rate the performance as "Good",the form would include the definition of "Good" as "Consistently outgoing and friendly; speaks to customers and makes them feel welcome," etc., etc. Does anyone have something they would be willing to share that might touch on something similar? It would be most appreciated.
