Need Performance Eval form for Fire & Police

Our Fire and Police Chiefs are always complaining that our current generic performance eval forms are just not effective for their special needs.

Does anyone know where I could find a good performance eval for assessing Police and Fire employees?

    I'm not sure what you want to measure; but I have designed an evaluation form that has 5 basic components and 10 specific questions within each area. Major areas: Job Knowledge, Communication skills, Decision making, Personnel/public relations, Leadership. I believe the key is to train the "evaluator" to be very honest in the rating. Also, I have separate inputs from the Supervisor, Peers, Subordinates, and self. It makes for intersting reading.
