Dress codes - body piercing-hair dying

I am currently reviewing a boilerplate employee manual put out by a publishing company with some alleged expertise in HR. So far it is pretty good. There is a paragraph that discusses dress code and says no to piercing particular body parts to include "nose, tongue, eyebrows, lips" and also says no to "unusual hair colorations or extreme makeup techniques." The company business has 75% of its employees making deliveries to hospitals and nursing homes and would like to include this paragraph. My question is: Is this still acceptable and if so, can it apply to all employees or just those who represent the company outside of the facilities. Thanks in advance.
