We just completed an audit of our I-9 forms and discovered that an employee who was hired in 1988 does not have an I-9 form. We don't know what happened to it since there has been personnel changes in the HR Department since then. Our only assumption is because this employee has had 3 different last names since her employment began due to marriage/divorce, that it was mistakenly shredded.

I am not sure if we need to do anything to remedy this. Any suggestions?

Exactly. And don't backdate signatures. Use accurate dates in each field of the form, including hire date. I would also write at the top, "Unable to locate original", today's date, your initials.
