Need examples of evaluations!

Would anyone be willing to share their performance evaluations with me? We are looking to revamp our existing evaluations and want to see what everyone else does. Looking specifically for management evaluations, but any evaluations will do!

I faxed copies of our part-time and full-time annual evals and also our 30, 60 and 90 day forms.

These were created in WordPerfect format and the formatting will not translate well to Word.
