My standard advice is don't use a standard form arbitration policy. It should be carefully tailored to the company and its needs.
That being said, if you go to the website of the American Arbitration Association (adr.org), you may be able to find some sample policies that they have published.
The decision to use arbitration should be carefully considered by the employer (considering both the pluses and the minuses).
