Can someone please give me some information on ERISA? We have just signed up with a company for Employee Life insurance. They are asking for an ERISA number. I know what ERISA means but I do not know where to get one or even who to contact.

I have 125 total employees and those employees are broken into 6 different federal id numbers. The parent company has 35 employees in it.

I would appreciate any information or direction.

    This is probably the plan ID number that is used for reporting on the annual 5500. It is a 3-digit number. Most welfare (health/life, etc) plans start with a 5. For ex, 501, 502, 503, etc. Retirement plans also use a plan ID number, as do 125 plans. Basically, any plan that files a 5500 needs one so the Dept of Labor can keep all your plans straight.

    I believe that you assign the number internally whenever you create a new type of benefit plan that requires filing with Dept of Labor.

    You can also check with your CFO or Finance department. They might have the number on file if they do your 5500 reporting.
