Need sample acknowledgement of sexual harassment policy

I'm conducting a sexual harassment policy and will have all employees sign an acknowledgement that they have received and will abide by our harassment policy. Does anyone have samples that they could email or fax to (256) 533-4805? Many thanks.

I will fax you the acknowledgement form that we use. It's simple but it serves the purpose. Let me know if you have questions. We used this when we did a mass training for employees and supervisors and we use this every week in orientation.
