Hepatitis B series vaccine

We are a CA healthcare provider and we are required to offer the Hepatitis B vaccine series to certain employees in high-risk areas (Nursing, Housekeeping) and am in need of a copy of the initial notification of this right and then a consent/decline for the employees to sign. I need help accessing these forms and have been unsuccessful so far...

I would like to send you the form that we use for our staff. We are a Public Hospital and also have to be sure "clinical" staff are offered this vaccine. Let me know your fax # and I will fax it to you.
