Retroactive Pay

Our County owe's an employee back wages. I thought we should have this employee sign a statement that he is accepting the payment as payment in full and the County will have no further liability for these owed wages. Is there any type of such statement any of you have had to prepare before, what should it say or all include? Thanks!
