You can provide utilization reports as long as there are no names or any other way for the receiver to identify who the information belongs to. The PHI can not be indentifiable. Hope this helps.

It is our internal policy to let ees attend dx visits and or other appointments without docking their pay for salaried ees. We trust that this policy will not be abused, and it has not for the last 4 years.

For I-9 purposes you can not request specific documents from the list provided. This should be indicated at the top of the I-9 form. However, you can request the ss# for payroll tax purposes.
