Medical Leave (Non FMLA)

One of my clients is not required to offer FMLA (<50 employees within 75 miles). They however want to offer a medical leave policy but cannot afford to offer the full 12 weeks. Is there a good rule of thumb or some other standard that could be applied in this situation?

I would not develop any type of policy that would make it sound too easy, too automatic, or too indifferent to the type of job the employee has and how difficult it would be to get along without him/her.
