Paul: Many insurance carriers will furnish an annual benefits statement to you as a service. This may be helpful if you have a large amount of employees and don't have time to process this information yourself. You may want to check with them before you tackle this project yourself.

We include a statement as well (in letter format) that accompanies the W2 statement. Ours includes unemployment and ER Fica/Mcare tax portions as well.
I have never seen something like that included in a persons paycheck but I have done them separately on a semi-annual basis. My husband gets one every quarter from his employer. In the one I did, I included a brief summary of the benefit and what dollar value the beneit held along with their annualized salary (based on normal scheduled hours). At the end I totalled everything to show them their true compensation. I was somewhat elementary in the one I did because of the make-up of the workforce, so I would suggest to take that into consideration when compiling this information - make sure everyone will be able to understand what you are trying to tell them. By the way, Benefits Expert from ADP has a component that will do this statement for you - it's tied directly in to payroll so it reads the employee's wage compensation, plus it knows all the benefits enrollment info including employee and employer premiums, so it can show what the employee "earns" in addition to wages. I promise I am not a salesperson for ADP. We are looking in to it right now - it looks great but expensive.
We also provide a substantial childcare discount (we are childcare/preschool) which we consider a part of each participating employee's compensation. I'd like to find a way to build this in to the statement too. Some employees receive as much as $13,000/year in discounts.
I'll be back in the office Monday. Am replying to this post to "push" it back to the top of this section so I don't forget you on Monday. I am ready to commit pending my boss's approval. ADP has been very flexible in the negotiating process!
