Updating Cobra Notices

Does anyone have any cobra notices they would be willing to share with me. I am updating my notices for compliance and could sure use some help! Appreciate any responses.

We have a decent one that was provided to us by our insurance carrier. Since I don't think we cann attach documents to this system suggest you send me an e-mail at [email]wstephan@kingsystems.com[/email] and I'll attach the notice for your use.
