COBRA Dependent Notification

I know I read this somewhere on the forum recently, but since I didn't need it then... Someone posted that they send COBRA notifications to Chris Former Employee and Dependents and get a proof of mailing from the post office. What is the offical title of the proof of mailing and what it the process to attain one. Thanks.

  • We also use proof of mailings when we send COBRA. You can pick up a pile of them at your local post office. They are white slips about 3"x5" that say "Certificate of Mailing" at the top and PS Form 3817 at the bottom. Costs us $.90 in addition to postage. On the slip, you write your return address and the address you are sending to. Hand both the slip and the letter to the postal clerk, who will then stamp the slip and toss the letter in the mail. You keep the slip as proof (I usually staple it right to my copies of what was sent).
