Does anyone have an example or a copy of a plan document for a Cafeteria Plan that you could email or fax to me? We would like to start a cafeteria plan and I need help in preparing the plan document.

Because of the legal technicalities of these types of plans, I don't think it is practical to just copy someone else's plan document. It may not fit your situation. I recommend consulting your benefits attorney or consultant who can tailor the plan to your needs. In most cases, you will use a TPA to handle your plan. I would suggest contacting a TPA (or several) and asking them for examples.

Pain:
Be careful about trying to use someone else's plan as a template. Sec 125 plans are different and usually employer specific. Trying to use someone else's plan may give you more PAIN than you're now experiencing. Your TPA or a benefits consulting firm can help you with this very easily.
