Job Descriptions and ADA

I am in the process of re-writing all of our job descriptions. Right now, they do not make reference to any physical demands of the positions. Does anybody have a general form they send to their supervisors to fill out that ask the questions needed to add to the job descriptions (ex. must stand for x number of hours per day, must be able to lift x number of lbs. occasionally/frequently)?

I am a vocational specialist and visit different employers on a regular basis for the specific purpose of providing a job description with the emphasis on physical demands. We typically provide these to doctors in worker's comp. cases. I have a few forms, from basic to extremely detailed.
