I was wondering how many of you require your employee to have the physician fill out the Certification of Health Care Provider form for FMLA when the leave is for the care of a parent or child?
100% of the time. I don't know of any other way to arrive at an approval or denial of the leave request.

Even if an eligible person is placed in f/t care, the family member is still able to take FMLA. Emotional support counts.
