Internship

We have a young man who is a full time college student who we would like to bring on for an internship. It would be unpaid he just wants to get experience in the industry that will help him with school and finding a job upon graduation. I would like to have him intern with us for 90 days at which time we would evaluate his performance and based on the evaluation we would employ him on a part time basis or not extend an offer. He agrees that this is what he wants to do.

Is there any type of paperwork I need to have him fill out (other than an internship agreement)or any rules that I need to follow? He is our first intern and I just want to make sure I keep on the straight and narrow!
