We did away with Post Offer Physicals about six months ago. It has come to be a bad move. (We've hired several people with back problems that we should have been keeping an eye out for...limiting some of what they were doing. Or getting a statement from their physician stating that they can do the job that they are hired to do.) Now we have several Workers' Comp. issues.) We're going back to implementing the physicals. We went back to the forms that we were using and do not like them. Does anyone out there have a questionnaire and/or physical form for post-offer/pre-employment physicals??? I do work in a health care facility and we do the physicals ourselves. I'm hoping some of you out there have these forms that you give your company's doctor or that you use in-house.

The form that Betty sent to me was a form that you would use for each position that you have where you would evaluate what actions were essential job functions (standing, walking, kneeling, etc.)
